Osteoarthritis (OA) is the most common form of arthritis and affects more than 20 million Americans. It is a condition that affects hyaline articular cartilage, the tough gristle that caps the ends of long bones.

Hyaline cartilage consists of a matrix made up of a combination of proteoglycans (complexes of proteins and sugars) and chondrocytes. Chondrocytes are cartilage cells that manufacture matrix under normal circumstances. They are responsible for nourishing the matrix as well.

However, when OA develops, a distinct change in the milieu of the joint environment occurs. Chondrocytes begin to elaborate destructive enzymes causing cracks in the cartilage, the synovium (lining of the joint) becomes inflamed, and the underlying bone becomes hard and forms spurs.

What causes OA to develop is usually injury or trauma. In any event an injury to the joint appears to be the inciting factor. Genetics play a role as well.

As OA progresses, biomechanical factors come into play. And this is where obesity plays a huge role in the worsening of disease. While overloading of the joint is the obvious major consideration, there is now abundant evidence that fat itself acts as an "organ" that perpetuates inflammation.

"Adipokines" are chemical messengers produced mainly by adipocytes (fat cells). The major adipokine is a substance called leptin. Leptin is interesting because it has contradictory actions. While it appears to help with the synthesis of growth factors that stimulate cartilage growth, it also appears to lead to the production of inflammatory proteins that cause further cartilage deterioration.

But that's not all. There are other adipokines that make a significant contribution in this cascade of events. Adipopnectin also seems to promote inflammation in arthritis. It causes the cartilage matrix to degrade and also promotes the manufacture of destructive enzymes.

Visfatin levels are also associated with cartilage degradation. Resistin promotes inflammation.

These fat produced proteins may explain why symptoms in people with OA who are overweight seem to get better with loss of body fat rather than just with weight reduction.

The bottom line is that OA is more than just a mechanical disease. Abnormal fat metabolism appears to lead to the production of factors that cause further cartilage loss and deterioration.

These new developments confirm the long held belief that people with symptomatic OA who carry excess fat may benefit from fat reduction.

These findings have been substantiated in real life clinical trials. Richette and colleagues (Richette P, et al. Ann Rheum Dis. 2011; 70: 139-140) demonstrated that obese individuals with knee OA who underwent gastric banding surgery not only had improvements in their symptoms but also blood levels of adipokines plummeted along with other inflammatory markers.

Chronic and recurring gout may even lead up to kidney failure and ultimately kidney stones. If you have any of these symptoms or signs, especially if you experience signs of a gout attack, then you should rush to a nearby doctor immediately. These symptoms of gout attacks include the following.


  • Chills, or fever rising and falling.

  • A quick and hasty onset usually between 12-24 hours and often happening and taking place overnight during sleep.

  • The pain comes and goes quickly.

If any of you out there experience any of these symptoms and signs of a gout attack, quickly head to your nearest doctor and report that you are experiencing a gout attack.

Knowing about gout symptoms may not be essential information to carry around as stock knowledge, but you never know when gout is going to strike you or any of your family members, relatives, loved ones or friends. What is considered an emerging area for arthritis therapy is the use of pulsed electromagnetic field (PEMF) therapy and electrotherapeutics. Termed, "electroceuticals", PEMF are highly-refined electromagnetic fields that are now being used to non-invasively "kickstart" the body's natural anti-inflammatory response to treat pain and inflammation and help soft tissue wounds heal faster.

Numerous in vitro and clinical studies have demonstrated that electric and magnetic energy has a positive effect on connective tissue healing. This has prompted an extensive amount of research in orthopedics and rheumatology. While modern pulsed electromagnetic fields (PEMF) have been available for more than 20 years, they are only now becoming a standard of care for delayed union fracture treatment.

Electromagnetic fields can be delivered by using direct placement of an electrode non-invasively by capacitive coupling (opposing electrodes placed on skin across the target area) or inductive coupling (PEMF induce an electric current in the target area without skin contact altogether).

In a randomized trial, Zizic et al studied a pulsed electric device used to treat 78 patients with chronic knee osteoarthritis via skin surface electrodes. The active treatment was superior to placebo in symptom reduction and was proved to be effective for symptom reduction and cost effective (Zizic TM, Hoffman KC, Holt PA, Hungerford DS, O'Dell JR, Jacobs MA, Lewis CG, Deal CL, Caldwell JR, Cholewczynski JG, et al. The treatment of osteoarthritis of the knee with pulsed electrical stimulation. J Rheumatol 22(9):1757-61, 1995).

The application of PEMF stimulation for osteoarthritis has been supported by additional work by Trock et al. In their experiment 86 patients with knee osteoarthritis were exposed to 9 hours of PEMF stimulation over a 1-month period using a non-contact device. Up to 36% of the patients noticed improvement in pain and function. PEMF therapy is considered safe but should be avoided in those who are pregnant, those patients with permanent pacemakers and patients with known cancer (Trock DH, Bollet AJ, Markoll R. The effect of pulsed electromagnetic fields in the treatment of osteoarthritisof the knee and cervical spine. Report of randomized, double blind, placebo controlled trials. Trock DH, Bollet AJ, Markoll R. Rheumatol 21(10):1903-11, 1994).

Because of the non invasive nature of this mode of therapy, it has promise as an additional weapon in the arsenal of the rheumatologist in treating osteoarthritis. Where PEMF will fall exactly in the sequence of treatment is still unknown. Further experience should clarify this issue.

You've heard the expression "you are what you eat"; but unfortunately that is the case with gout symptoms and arthritis symptoms. The good news is that there are a healthy number of delicious foods that will combat both of these. True, there are many delicious foods that you will need to steer clear of; but hopefully, with enough of the proper foods in you (and maybe a little splurge now and then...very little) you can reach a happy medium.

How to relieve gout symptoms

Eating the right foods is just as important as staying away from the wrong foods. A diet high in omega 3 fatty acids goes a long way to decrease gout symptoms; but the fats from dairy products work in just the opposite way. Certain veggies are on the 'to- be-avoided' list but overall; celery, carrots and almost all citrus fruits have pectin in them. Oh and by the way...

Symptoms of arthritis are actually treated with most of the foods used to treat gout. Onions, for instance, are one of the healthiest vegetables you can eat. Not only are they beneficial to sufferers of rheumatoid arthritis and an essential part of a gout diet; but they contain strong antibiotic qualities and are good for sufferers of cardiac issues as well. If you could get the vitamin C necessary for the treatment of gout symptoms and an analgesic to help relieve symptoms of arthritis and one tasty food, would you eat it? Of course you would.

Fortunately, this is exactly the case with sweet cherries. No, not the Maraschino cherries that you drop in your old-fashioned (cocktail)! God only knows what's been done to those. The fresh sweet cherries are what we are referring to. Sweet cherries are not your cup of tea? Just like with humans, when a your kitty becomes a senior she is prone to typical disorders of aging cats. If she can't jump up on your lap like she used to then she may have feline arthritis. Arthritis affects the joints, causing stiffness and pain. You can then work with hot and cold to find the particular combination to treat your specific symptoms of arthritis.

This therapy works to improve the body's own healing abilities. Heat allows dilation of the blood vessels. This in turn promotes circulation, resulting to reduced spasm of the muscles and increased oxygenation. Heat also defers the sensation of pain. Dry heat, such as lamps and pads, can be used as well as moist heat through hot baths, or wash cloths soaked in hot water.

According to the American College of Rheumatology, 5 to 10-minute ice massages applied to a painful area within the first 48 hours of pain onset can help relieve pain while heat should be used for pains that last longer than 48 hours.

When using heat therapy, you have to be careful with the heat being applied to prevent burning the skin. You want it hot, but not hot enough to be uncomfortable. Also, many people with arthritis find soaking in hot baths and saunas effective relief for arthritis pain. Heat packs applied to arthritic joints at least twice daily has been proven to relieve pain and stiffness.

While heat opens blood vessels, cold constricts the blood vessels. Cold therapy can work on swollen parts of the body by producing a dulling and numbing effect and also reducing the swelling of inflamed joints. Ice packs are most effective during arthritis flares and can be effective when applied over joints. The cooling sensation of the skin above the affected areas lessens spasm of the underlying muscles, which improves the threshold of pain. Cold packs can easily be done by filling zip-lock baggies with water and placing them on the freezer to chill. For some people with arthritis, cold therapy may be enough to completely ease pain. However, many people choose to alternate hot and cold therapy.

Hyaline cartilage is made up of a matrix consisting of a combination of proteoglycans (complexes of proteins and sugars) and chondrocytes. Chondrocytes are cartilage cells that manufacture matrix under normal healthy circumstances. They are responsible for nourishing the matrix as well.

However, when OA develops, a distinct change in the joint environment occurs. Chondrocytes begin to elaborate destructive enzymes causing cracks in the cartilage. These are called "fibrillations."

One of the most common joints affected by osteoarthritis is the knee. This is not a surprise since OA preferentially attacks weight-bearing joints.

The treatment of OA of the knee is primarily symptomatic. Weight loss, exercise, physical therapy, assistive devices (such as canes and walkers), analgesics (pain-relieving medicines), non-steroidal anti-inflammatory drugs (NSAIDS) which also reduce inflammation and pain, as well as thermal modalities such as heat and ice can all be employed.

But what happens when the patient doesn't respond to these?

The next step is injection therapy. There are two main forms of injection therapy used for OA of the knee. The first are corticosteroid injections. The other major type, hyaluronic acid injections will be discussed in another article.

Corticosteroid (another term used is glucocorticoid - I will use these terms interchangeably) or glucocorticoid injections are used to provide rapid short term relief from the pain and inflammation caused by OA of the knee. Corticosteroids - also known as "cortisone" reduce inflammation and theoretically reduce potential damage to the joint. They are administered in combination with a local anesthetic to help provide immediate relief of pain until the corticosteroid has a chance to "kick in." These injections are administered using sterile technique with a local anesthetic. It's critical that ultrasound guidance be used in order to ensure accuracy. If the corticosteroid isn't placed properly in the joint space, it will not provide optimal relief. They should not be administered more often than three times per year since there is evidence that corticosteroids given more often can damage cartilage.

While generally safe, corticosteroid injections can cause some minor side effects such as flushing of the face, elevation of blood sugar in patients with diabetes, skin atrophy at the site of injection, a flare of pain the evening of the steroid injection, and sometimes some local swelling. Infections are rare if proper technique is used.

Caution should be exercised when giving these injections to patients on anticoagulants such as warfarin. Corticosteroid injections provide short term benefit. The length of effectiveness can range anywhere from weeks to several months. On occasion, they will help for a longer period of time.

Gout is a common metabolic disease characterized by elevated blood levels of uric acid (UA). This arises as a result of either the inability to excrete (get rid of) UA or overproducing it. In any event, what occurs is that over time, deposits of monosodium urate form inside joints and other organs, particularly the kidneys, which is the major organ responsible for excreting UA.

At a certain critical point in time, if enough UA has built up, an attack of gouty arthritis (GA) will occur.

GA typically affects a joint or joints in the leg, preferentially attacking the foot, ankle, or knee. The attacks tend to occur early in the morning, often awaking a patient from sleep. The pain builds in intensity and is accompanied by swelling, redness, and heat. The discomfort is so severe that the patient may complain that even "the weight of a bedsheet" on the affected area causes unbearable pain.

The treatment of GA is generally divided into treatment for the acute attack and treatment of chronic disease.

What is Gout?

Gout is a type of inflammatory arthritis that occurs in response to a build-up of crystals that are deposited in joints, soft tissue or bone. The crystals are formed as a byproduct of purine breakdown that occur naturally in the body. About 5.1 million Americans are affected by gout. It affects men older than 40 more than women, although after menopause, both men and women are equally affected.

What Causes Gout?

Uric acid is a natural byproduct of purine breakdown which is then eliminated by our kidneys. Our bodies are not very efficient however in eliminating uric acid and thus a build-up occurs which is known as hyperuricemia. Hyperuricemia is the precursor to gout although not everyone who has hyperuricemia will get gout for reasons that are not well understood. This uric acid leads to crystal formations that are deposited in joints, bones and soft tissue. These needle-like crystals can cause excruciating pain that is often likened to breaking a bone.

4 Stages of Gout

1. Asymptomatic hyperuricemia

During this period, the person has no symptoms of gout but none-the-less, crystals are deposited and accumulate which then leads to a gout attack. Again, why this occurs in some people with hyperuricemia and not others is not well understood.

2. Acute gout attack

Left untreated, an acute attack resolves spontaneously in 3 to 10 days. It is believed that the inflammation and lower pH that occurs during the attack eventually causes the crystals to dissolve. In addition, some of the crystals become walled-off in tissue while other types of cells known as macrophages may elicit an anti-inflammatory response. Finally, specific proteins are thought to coat the crystals which also suppress inflammation. These processes lead to a resolution of symptoms within 3-10 days.

3. Intercritical period

This is the period between attacks. Generally, about 68% of people will experience another gout attack or flare within the first year, 78% within 2 years and 84% within 3 years. A small minority will never experience another gout attack. During this intercritical period the individual remains pain free despite the fact that damage to the underlying structures can still occur.

4. Advanced Gout

Over time, the persistence of crystals cause chronic low-level inflammation that leads to joint damage and build up of crystal deposits that are known as tophi. These deposits lead to joint erosion and deformities that are clearly evident on X-ray or MRI.

How is Gout Diagnosed?

The gold standard for diagnosis is to take a sample of joint fluid during an acute flare to analyze it for monosodium urate crystals. However this is not always practicable during a medical exam. If the joint is not aspirated, a very detailed history is taken, the affected joint is carefully examined and blood work is ordered because gout can mimic other diseases.That is why it is very important to see your provider during an acute attack.

How is Gout Treated?

During an acute flare, treatment focuses on eliminating pain and disability as quickly as possible. To this end several types of medications can be used. Since inflammation is an underlying condition, medications that target inflammation are often used.
